Getting ready for the first day home
You can do most of this before your loved one leaves the hospital. None of it needs an account.

Before you leave
Six things to ask at the hospital
- Put your name on the record. Ask the nurse to write your name down as the family caregiver.
- Get the medicine list. Ask which medicines are new, which ones changed, and which old ones to stop.
- Learn the warning signs. Ask what to watch for, and who to call about each one, day and night.
- Book the follow-up visit. Book it before you leave, and ask how soon it should be.
- Ask about help at home. Ask what the doctor has ordered, such as a visiting nurse, therapy or equipment, and who will call you to set it up.
- Get it in your language. Ask for the papers in the language your family reads best.

A helper
Ask about a helper
Our helpers work with our hospital partners. Tell us your city and when your loved one is coming home. If a helper works near you, we will call you. If not, we will send you the pages and tips for the first month.

Tell your hospital
If you think your hospital should offer this, tell the discharge planner or the patient advocate, and send them our page for hospitals.
